- Tackle and Cheer Only - 2026 Physical (January 1, 2026 or sooner). If your 2026 physical isn't scheduled until the summer, please download the Medical Clearance Form below. This will need to be completed by your pediatrician prior to registering.
- Tackle, Cheer, and Flag - Birth Certificate.
- Tackle, Cheer, and Flag Coaches - CORI Form. Download below. (Note - There is a selection process for coaches)
- Tackle Coaches - Complete the coach certifications. Links below. Email certificates to info@qyfca.com.
