REQUIRED FORMS

The Bay State AYF requires a couple forms in addition to the electronic waivers for all Tackle and Cheer participants.

These must be printed, completed, signed/initialed and returned to the team.


Required Forms:


1. PAGE 2 & 7 of the PRINT FORMS packet must  be completed, signed and initialed.  These forms can be found by logging into the registration site and clicking PRINT FORMS.  They are also attached below.


2. A PHYSICAL or MEDICAL CLEARNACE FORM dated 1/1/2022 or sooner.  If your child's physical is scheduled beyond the first week of August then please have your doctor sign the medical clearance form which is also found in the PRINT FORMS packet on the registration site, as well as attached below.


3. Your child's FINAL REPORT CARD for the 2021-2022 school year.


All of these forms are required prior to the start of practice.


Forms can be either mailed to us at QYFCA, PO Box 133, Quincy, MA 02171 or you can scan/take a photo and email to info@qyfca.com.
