REQUIRED FORMS

The Bay State AYF requires a couple forms in addition to the electronic waivers for all Tackle and Cheer participants.

These must be printed, completed, signed/initialed and returned to the team.

Required Forms:

1. A PHYSICAL or MEDICAL CLEARANCE FORM dated after 1/1/2023.  

If your child's physical is scheduled beyond the first week of August 

then please have your doctor sign the medical clearance form,

 which can be downloaded from this page.

2. Your child's FINAL REPORT CARD for the 2022 - 2023 school year.

All of these forms are required prior to the start of practice.
