REQUIRED FORMS

The Bay State AYF requires a couple forms in addition to the electronic waivers for all Tackle and Cheer participants.

These must be printed, completed, signed/initialed and returned to the team.


MANDATORY FORMS


1. Signed and initialed Participation Form and Emergency Medical Clearance

Form. Instructions for how to download these forms is in the link below.


2. A PHYSICAL or MEDICAL CLEARANCE FORM dated after 1/2/2025 or sooner.

If your child's physical is scheduled beyond mid July 

then please have your doctor sign the medical clearance form,

 which can be downloaded from this page.


3. Your child's FINAL REPORT CARD for the 2024-2025 school year.


All of these forms are required prior to the start of practice.
